• Read SMHS and SMUSD Required Readings
• Download the fill-able PDF file titled "Critical Registration Forms" below
  
• Type in your responses on this form
• Print the document RIGHT AWAY – your responses will not be saved
• Hand sign every page where required.
• Staple your checks to their respective forms. (Titan Package, PTSA Membership and Donation Form, SMHS Foundation Annual Campaign)
• Return the forms along with the other forms from this First Day Packet by mail or by dropping them off at the APO Office before August 15th
• Thank you for helping us with transitioning to a more dynamic and accurate Registration Process.
